2. Services Description
LEMN Care provides professional in-home care services including:
•Personal care assistance (bathing, dressing, grooming)
•Homemaking and meal preparation
•Medication reminders and safety monitoring
•Transportation and errand assistance
•Companionship and emotional support
All services are provided by trained, qualified caregivers under the supervision of LEMN Care management and in compliance with Michigan state regulations.
3. Client Responsibilities
As a client, you agree to:
•Provide accurate and complete health information
•Maintain a safe environment for caregivers to work
•Treat caregivers with respect and dignity
•Notify us promptly of any changes in care needs or schedule
•Provide timely payment for services (if private pay)
•Not request caregivers to perform tasks outside their scope of practice
•Follow all care plan instructions and recommendations
4. Medicaid-Approved Services
LEMN Care is a licensed Medicaid provider. For Medicaid-covered services:
•You must be eligible and approved for Medicaid home care benefits
•Services must be authorized by your Medicaid care coordinator
•Care hours are subject to Medicaid guidelines and limitations
•We will bill Medicaid directly for covered services
•Additional services beyond Medicaid coverage may require private payment
5. Scheduling and Cancellations
Scheduling:
•We will make reasonable efforts to accommodate your preferred schedule
•Schedule changes require at least 24 hours notice when possible
•We reserve the right to modify caregiver assignments as needed
Cancellations:
•Please provide at least 24 hours notice for cancellations
•Repeated last-minute cancellations may affect service availability
•Emergency cancellations will be handled on a case-by-case basis
6. Payment Terms
For private pay clients (non-Medicaid):
•Payment is due as agreed upon in your service agreement
•We accept cash, checks, and major credit cards
•Late payments may incur additional fees
•Services may be suspended for non-payment after proper notice
•Minimum service hours may apply for certain services
7. Limitation of Liability
While we strive to provide the highest quality care:
•We are not responsible for outcomes beyond our reasonable control
•Our caregivers are not medical professionals unless specifically stated
•We do not provide emergency medical services or skilled nursing care
•Liability is limited to the extent permitted by Michigan law
Emergency Situations: In case of medical emergency, call 911 immediately. Our caregivers are trained to assist but are not substitutes for emergency medical services.
8. Termination of Services
Either party may terminate services with written notice:
•Clients may terminate services at any time with 14 days notice
•LEMN Care may terminate services with 30 days notice
•Immediate termination may occur for safety concerns, non-payment, or abusive behavior
•Outstanding balances remain due after termination
9. Privacy and Confidentiality
We are committed to protecting your privacy and maintaining confidentiality of your personal and health information in accordance with HIPAA and our Privacy Policy.
•All caregivers sign confidentiality agreements
•Your information is stored securely and accessed only by authorized personnel
•We will not share your information without your consent, except as required by law
10. Website Use and Intellectual Property
Our website and all content, including text, graphics, logos, and software, are the property of LEMN Care and protected by copyright laws.

•Unauthorized use may violate copyright, trademark, and other laws
•We reserve the right to modify or discontinue the website at any time
11. Governing Law
These Terms of Service are governed by the laws of the State of Michigan. Any disputes arising from these terms or our services will be resolved in the courts of Oakland County, Michigan.

We reserve the right to modify these Terms of Service at any time. Changes will be effective immediately upon posting to our website. Your continued use of our services constitutes acceptance of the updated terms.
